Finished Object Entry Form Instructions

The username you choose for this MAL can be any unique identifier you would like to use.  Please use the SAME username each time you submit this Finished Object Entry form or a Theme Submission form.

 

Your email address will only be used in case I need to reach you for clarification of a submission or to let you know you won a prize.

 

If you need to correct a previously submitted Finished Object Entry, please DO NOT submit a second form for the same project. Instead, please email me at correction@fibernymphdyeworks.com and let me know what corrections need to be made.
